Consent and Release

I consent to participate in the production of media which may record, use, and/or share my likeness, first and last name, image, voice, performance, and visual works which may be personally identifiable to the general public during any events, programs, or activities—whether in video, photograph, audio, or written format—for church-related purposes (“Media”). Consent is granted to publish Media in any and all formats, including photographs, video recordings, audio recordings and digital recordings. I furthermore understand and consent to the possibility that I may be contacted multiple times by Victory Church in relation to Victory Church’s media production/efforts.

I understand and agree that these recordings, photographs and other media may be used and disclosed without compensation to me. I hereby grant Victory Church and its affiliates, successors, and assigns the worldwide right to use the Media for reasons that may include, and will not exceed the following: use during services, on social media, Victory Church websites, and other published materials and media platforms. I hereby waive my right to approve and/or inspect any Media in which I, or my likeness, appear and acknowledge that any Media may be edited for clarity or length.   

I hereby acknowledge that my participation is voluntary and personal information may be shared. In the event any personal information shared contains any suicidal thoughts or actions, I hereby acknowledge that Victory Church may contact and/or notify the proper authorities to assist me in receiving the proper support and care, including the provisions of Georgia’s Mental Health Code (Title 37, Chapter 3). Under Georgia’s Mandated Reporter Law (O.C.G.A. § 19-7-5), Victory Church must immediately report suspected child abuse or imminent risk of serious harm to the physical or mental health of a child to the Division of Family and Children Services (DFCS).

I acknowledge that Victory Church will own all rights, title, and interest in such Media and may edit, reproduce, or distribute in any legal manner, in keeping with the Privacy Policy and Terms of Service as posted on victoryatl.com and detailed herein. I furthermore consent to the Privacy Policy and Terms of Services as posted in their current version on victoryatl.com.  I release, discharge, and hold harmless Victory Church, its staff, and representatives from any and all from any and all causes of action, suits, claims, rights, damages, losses, costs, expenses (including attorneys’ fees and costs incurred), contracts, agreements or controversies of any nature whatsoever, known or unknown, liquidated or unliquidated, suspected or unsuspected, fixed or contingent in law or in equity that I may have, including, but not limited to, claims based upon defamation, invasion of privacy, or rights of publicity arising from the use of Media as described herein.

I acknowledge that this consent is considered indefinite but may be revoked at any time and that such revocation will entail the deletion of my contact information by Victory Church. To opt-out of this consent and request deletion of your information, please do so by submitting a written request to comms@victoryatl.com.
