Financial Assistance Form

IMPORTANT INFORMATION: BEFORE PROCEEDING TO APPLICATION, PLEASE READ:

Victory World Church has a heart to help and serve our community. However, due to the significant impact of our current economy and the high volume of requests from our congregation, we are only potentially able to provide financial assistance to those actively connected to Victory through:
- Membership/regular attendance of weekend services for at least 3 months
- Serving others through one of the ministries of Victory World Church
- Participation in a Victory small group

If you feel you meet the above criteria, please download Request for Financial Assistance. Please note that meeting the criteria above does not guarantee assistance.

IMPORTANT:
In order for your application to be considered, you must submit copies of your drivers's license, all current bills and, if requesting rent assistance, your current lease. (If you pay your rent to an individual we need the landlord to complete and sign a W9 form and email or fax to us).

The names and addresses on your bills and on your license must match the address provided on your application.

- in person at our Administrative Offices.


*Regardless of the urgency of your request, your application will not be processed until all accompanying documentation is received.

Once we have received your documentation (Financial Assistance form, Drivers License, copies of all bills,
DO NOT send Social Security Information) please be patient. Due to the volume of applications we receive, it could take up to 7-10 business days to process your request. In the meantime, we encourage you to download the Citywide Resource List from the Pastoral Care page on our website for more options.
